For the first time, "can my watch check my blood pressure" has real answers instead of vaporware. Apple ships FDA-cleared hypertension notifications on Apple Watch Series 9 and later and Ultra 2 and later: a background algorithm that watches how your blood vessels respond to your heartbeat over 30-day windows and warns you if it sees consistent signs of high blood pressure. Samsung went a different way: since March 31, 2026, Galaxy Watch 4 and newer can display actual systolic and diastolic numbers in the US, with a catch large enough to be the headline.
The honest frame: Apple's feature is a screening net, not a measurement, and it deliberately tells you nothing if you already have a hypertension diagnosis. Samsung's numbers require recalibrating against a real arm cuff every 28 days and are officially a wellness feature, not a medical one. Neither replaces a $30 validated cuff, which remains the only accurate way to know your numbers. And the ground may shift again on September 9, when Apple's next watches arrive with another blood-pressure-related feature reportedly in regulatory review.
Why Wrists and Blood Pressure Are Hard
Clinical blood pressure measurement squeezes an artery until flow stops, then listens as it returns. A watch cannot do that. What it can do is optical: shine light into your wrist and analyze the shape and timing of each pulse wave as blood vessels respond to heartbeats. From those patterns, algorithms estimate pressure or detect the signatures of chronically elevated pressure.
That indirection is why every consumer implementation carries an asterisk. Pulse-wave analysis drifts with vessel stiffness, wrist position, skin temperature, and time, which is why Samsung anchors it to a monthly cuff calibration and why Apple chose not to show numbers at all. Roughly 47 percent of American adults have elevated blood pressure and a large share do not know it, which is exactly why an imperfect always-on screen on the wrist is still worth having, as long as you know what it is and is not telling you.
Apple: A Screening Net, Cleared by the FDA
Apple's hypertension notifications launched with the Series 11 in September 2025 and run on Series 9 and later and Ultra 2 and later with watchOS 26, in over 150 countries. The algorithm was built with training data spanning studies of more than 100,000 participants and validated in a clinical study of over 2,000, and it received FDA clearance days after announcement. Apple expects it to notify over 1 million people with undiagnosed hypertension in its first year.
The design choices are the story. It runs passively over 30-day periods, so there is nothing to do and no number to obsess over. When it fires, Apple's guidance is to log a week of readings with a third-party cuff and bring them to your clinician. And Apple is explicit about the limits: notifications will not catch all cases of hypertension, and the feature is not intended for people under 22, during pregnancy, or for anyone already diagnosed, whom it deliberately does not monitor. It is a net for the undiagnosed, not a management tool for the diagnosed.
Samsung: Real Numbers, Real Strings Attached
Samsung's approach gives you what Apple withholds: on-demand systolic and diastolic readings, on Galaxy Watch 4 and newer with Wear OS 4, through the Samsung Health Monitor app, rolling out in the US since March 31, 2026 after years of being limited to other markets.
The strings: you must calibrate against a standard upper-arm cuff at setup and again every 28 days, because pulse-wave estimates drift. You need a Samsung phone for the Health Monitor app. And in the US, Samsung positions the feature as wellness rather than medical, which is why it did not need FDA clearance. Translated: the readings are estimates anchored to your last calibration, useful for spotting trends and big deviations between cuff checks, and not something a clinician will treat as gospel. If you will actually do the monthly calibration, it is a genuinely useful trend tool. If you will not, its numbers quietly become fiction.
The Part Nobody Ships Yet
What does not exist in 2026: a cuffless watch that measures blood pressure accurately enough to diagnose or manage hypertension without calibration. Every honest engineer in this space says the same thing. Apple reportedly has more coming: another blood-pressure-related feature is said to be in regulatory review ahead of the September 9 event where the Series 12 and Ultra 4 are expected, though whether it needs new hardware is unclear. Until a regulator clears a cuffless measurement claim, treat any watch's numbers as estimates and keep a validated arm cuff in the drawer; it costs less than a watch band and it is still the instrument your diagnosis will actually rest on.
